- (a) Birth certificate under the Birth, Death and Marriages
Certification Act,1986.
(b) Hospital/Auxillary Nurse and Midwife(ANM) register
record.
(c) Anganwadi Record.
(d) Declaration of age of the child by the parent or guardian.
- Residence Proof-
- (a) Ration Card issued in the name of parents ( mother/father having name of the child.
(b) Domicile certificate of child or his/her parents.
(c) Voter I card of any of the parents.
(d) Electricity bill/mtnl telephone bill/water bill.
(e) Unique identify Card of mother/father/child issued by
government of India.
(f) Passport in the name of any of the parents or child.
- Photographs-
- (a) passport size of child-2
(b) stamp size of child -1
(c) passport size of child’s family with child- 1
- Documents to be submitted along with the form for Economically Weaker Section and Disadvantaged Category
- Birth proof -
- (a) Birth certificate under the Birth, Death and Marriages Certification Act,1986.

- (a) A certificate issued by a Revenue Officer not below the
rank of Tehsildar or any other Competent Authority.
(b) Caste Certificate issued by DC Office.
Disadvantaged Proof-
(a) Medical Cerificate from Govt. Hospital, in case of
children with special needs/ disabilities.
(b) A Certificate issued in accordance with instruction issued
by the Department of Women and Child Development,
GNCT of Delhi, vide order dated 11.12.12 in case of
Orphan Child.
- Application form to be filled with blue ink in English/Hindi ( if the form is in English /Hindi language)
- Incomplete Admission Form will not be accepted
