v  PHOTOCOPIES OF THE ORIGINALS TO BE SELF ATTESTED. 
v  IN CASE OF ANY DISCREPANCY FOUND, THE REGISTRATION STANDS CANCELLED.

of Proof of Residence (MTNL bill / Passport / Driving License / Voter I
Card / Electricity- Water Bill / Rent Agreement). Please submit the latest copy
of the bill.

of the birth certificate of the child issued by Municipal Corporation or
equivalent authority. 
v  In case of first born child- Affidavit
by the parents confirming that the child is their first born child duly
attested by Notary Public.

State Transfer/Overseas transfer- Attested
photocopy of the Transfer Order from the company. The transfer order
should not be more than one year.

Please bring the photocopy of latest
report card / ID card / latest fee slip (any one).

documentation and certificate from competent medical officer if child is
suffering with some medical ailment.
